AUTHORIZATION, CERTIFICATION AND CONSENT
I/We hereby certify that all information and statements in this application are correct and complete, and that they are made for the purpose of obtaining credit. I/We likewise certify that all signatures appearing herein are genuine.
I/We agree that this application and the documents I/we may provide shall remain as the property of Asia United Bank Corporation (“AUB”), whether credit is granted or not.
I/We hereby authorize AUB (a) to report and make disclosures of relevant information as may be necessary for AUB to comply with applicable anti-money laundering laws, rules and regulations, as well as to provide such information to the Anti-Money Laundering Council in compliance with Republic Act No. 9160 (as amended) and its rules and regulations, to other financial institutions, to the Credit Information Corporation, and to other credit bureaus or institutions, pursuant to Republic Act No. 9510 (as may be amended), its implementing rules and regulations, and the Manual of Regulations for Banks, (b) to conduct random verification with the Bureau of Internal Revenue as required under the Manual of Regulations for Banks and Revenue Regulations No. 4-2005 (as all such regulations and issuances may be updated, amended and/or supplemented from time to time), and accordingly waive on a continuing basis the confidentiality of account information as may be necessary to fully implement such authority, (c) to inquire into any of the deposit accounts or properties I/we maintain with AUB, as well as AUB’s parent companies, subsidiaries or affiliates (collectively, the “Group”), for the purpose of implementing the automatic debit authority and/or set-off provisions under the loan for which I/we hereby apply, and/or (d) to obtain from or disclose to the Group any pertinent information regarding me/us and/or my/our loan for purposes of credit evaluation and assessment of AUB/Group products and services deemed by AUB to be relevant to me/us that may be offered to or presently being provided to me/us, and (e) to obtain from the Credit Information Corporation and other credit bureaus or institutions, basic credit information on me/us. Moreover, I/we hereby authorize AUB to request and obtain information on the status of any court case to which I/we am/are a party and/or the authenticity and/or status of any permit, license or authorization granted to me/us by any third party, including government agencies and other regulatory bodies. I/We hereby hold AUB and its directors, officers, employees, stockholders and agents free and harmless for any liability arising from the authority herein granted unless shown by final judgment to have been exercised with gross negligence or fraud.
Further, I/we authorize AUB to share my/our personal data with the Group, as well as authorized agents or third parties that provide services or have contractual obligations with the Group, for the following purposes:
1. For AUB, members of the Group, and AUB’s accredited third party service providers and program partners: (i) to further secure me/us from fraud, unauthorized and illegal transactions, and anti-money laundering and terrorist financing risks, (ii) to provide me/us with a customized experience and/or to offer me/us products and services that are relevant and suitable to me/us, (iii) to participate in any automated teller machine network, electronic fund and/or credit card network; (iv) to validate, verify and/or update provided information and supporting documents, and (v) to comply with applicable laws, rules, regulations, contractual obligations, or orders of courts or quasi-judicial and administrative offices; and
2. As may be necessary in the settlement of disputes or claims regarding my/our account/s or in defense of AUB and/or any of its officers and personnel.
I/We recognize that AUB is committed to protect my/our personal data against unauthorized processing by complying with applicable laws, rules and regulations on data privacy and secrecy of bank deposits, such as but not limited to the Data Privacy Act of 2012 (Republic Act No. 10173) and its implementing rules and regulations, Law on Secrecy of Bank Deposits (Republic Act No. 1405), Foreign Currency Deposit Act (Republic Act No. 6426), and the General Banking Law of 2000 (Republic Act No. 8971).
I/We hereby confirm that I/we may view the Data Privacy Policy of AUB at any time, which includes a description of my/our rights as data subjects, at https://www.aub.com.ph.
I/We also understand that I/we may access, update or correct my/our personal data with AUB and/or withdraw my/our consent under this form by reaching the AUB Data Privacy Officer with the following contact information:

Finally, I/we hereby agree that if I/we withdraw my/our consent to the processing of my/our personal data, or if I/we terminate my/our relationship with AUB, AUB has a period of five (5) years within which to delete and securely dispose of my/our personal data, unless a longer retention period is required under applicable laws, rules and regulations and/or AUB’s information security policy.
